Supported Living

The Supported Independent Living (SIL) program is made up of services that are tailored to each participant’s unique needs. Our Intake Team meticulously reviews historical information and interviews each participant who joins the SIL program. Once the Intake process has been completed the Intake Team recommends the type of strengths that the participant will need in their team to be successful.

DAY
PROGRAM

Operating between 8:00 am and 4:00 pm, Monday through Friday, Day Program offers life skill-building activities, social skill-building opportunities, vocational skills training, volunteer opportunities and recreational options. These activities focus on skill-building while providing the participant with a sense of accomplishment, ownership, group membership and self-esteem.

The goals of this service are to:

  • Provide the exposure to skill-building and healthy activities during the daytime hours.
  • Provide support to residential service providers by engaging the participant in activities that impact healthy residential living.
  • Facilitate participation in ongoing vocational activities.
  • Assist in job skills training, resume and interview preparation.
  • Create opportunities for competitive paid employment for our participants through our Swifty’s Social Enterprise, including bed bug removal, lawn care, and snow removal services.
  • Create space for Indigenous cultural practice through our Elder such as sharing circles, rattle making, beadwork, medicine picking and more.

HARM REDUCTION PROGRAM

Turning Leaf’s Harm Reduction Program focuses on risk management initiatives, policies and practices that focus on minimizing the negative health, social, and economic consequences for people who are actively engaging in substance use, and at higher risk of experiencing associated harms. The Harm Reduction Program accepts that a continuing level of drug use is inevitable, and the primary goal is to empower people with the knowledge, information and resources to reduce/mitigate harms and increase their safety and health.

The measurable benefits of a Harm Reduction approach are:

  • Providing education
  • Increasing access to safer use supplies
  • Offering support on how to use safely
  • Increasing safe disposal options to diminish the number of discarded syringes on public/private properties, improving the safety of our communities

A Harm Reduction approach includes:

  • Conducting an imminent risk assessment
  • Developing a collaborative rapport between the participant and staff team
  • Identifying the major presenting issue and individuals who may be involved
  • Encouraging dialogue about the impact of the issue, including feelings about the presenting issue
  • Generating and exploring alternative coping strategies
  • Developing a safety and control plan
  • Restoring functioning by implementing a plan
  • Planning for follow-up contact
  • Completing ongoing reporting

Crisis Stabilization

At times, many of the people coming to Turning Leaf for help are at a place in life when their ability to cope with life’s stresses, their acute mental health symptoms, and problematic behaviours is compromised and no longer effective. This is when the Crisis Stabilization Team is called to help the participant in need.

The goal of the Crisis Stabilization team is to attend to and manage crisis scenarios 24 hours/day. This team is asked to support either new participants to Turning Leaf or those participants receiving residential treatment or community support but are unable to maintain emotional/behavioural stability while receiving service. The task is to assist these participants in achieving emotional and behavioural stability and reintegrate them back into regular residential or community services.

COMMUNITY
SUPPORT

Through our highly trained, skilled and compassionate Community Support Workers, participants are given access to treatment and support throughout the community. Support staff meet our participants where they are. Each participant’s needs, risks, challenges, goals, and strengths are unique and the Community Support team tailors treatment and support around the person.

The specific kinds of things our Community Support team do are innumerable and all of them invaluable. It can be anything from advocacy with legal issues, employment or living arrangements, to fundamental living habits such as meal planning, grocery shopping, basic hygiene and personal presentation. We help our participants develop money-management skills. We provide access to appropriate and healthy recreation and leisure options.

Primarily, the Community Support team’s goal is to engage the participant in a person-centered manner that allows for healthy, secure and trusting relationships to develop.
